Amiga: delete dead files and dead-macro code

Browse Source 
Delete amigst.c (empty) and amitty.c (TTY/BBS stub).  Drop WINVERS_AMIT,
SUPERBITMAP_MAP, EXTMENU, SHELL/dosh(), and the bbs_id reference in
src/files.c.
