nhmall a8313d47e5 rearrange some code in makedefs.c ...

This was a part of an earlier change that was reverted.
Get all the obsolete parts of makedefs arranged so that
they can be disabled simply by not defining
OLD_MAKEDEFS_OPTIONS.

Right now OLD_MAKEDEFS_OPTIONS is defined.
